It often erodes smaller veins in the body that it's administered by so many nurses now will refuse to start an IV for Vancomycin in anything smaller than a PICC-line (peripherally inserted central catheter) or a central line (a line inserted on the chest directly into the superior vena cava). |

| In spite of the fact it's a floor with primarily respiratory-compromised patients, none of the four inviti I was assigned had anything wrong with their lungs. One alcoholic with psych issues drying out, one very demented 66-year old with FTA (failure to thrive), one with end-stage renal disease and the last with inviti VRE (Vancomycin-resistant enterococcus) and dementia. And, here's a nice little gem. When I came on shift today, the patient with VRE's chart was missing. The residents had taken it on rounds. And weren't anywhere to be found. At this inviti point, neither the nurse coming off shift, my preceptor or myself knew that the patient had tested positive for VRE. The doctors had noted it in the chart, but took the chart with them. And though the patient was supposed to be on contact precautions because of the VRE, they hadn't had anyone put up a sign alerting people to the precautions which is standard practice or even uttered a word about it to anyone. |
