One alcoholic with psych issues drying natalie portman out, one very demented 66-year old with FTA (failure to thrive), one with end-stage renal disease and the last with VRE (Vancomycin-resistant enterococcus) and dementia. And, here's a nice little gem. When I came on shift today, the patient with VRE's chart was missing. The residents had taken it on rounds. And weren't anywhere to be found. At this point, neither the nurse coming off shift, my preceptor or myself knew that natalie portman the patient had tested positive for VRE. The doctors had noted it natalie portman in the chart, but took the chart with them. And though the patient was supposed to be on contact precautions because of the VRE, they hadn't had anyone put up a sign alerting people to the precautions which is standard practice or even uttered a word about it to anyone. So when I came on shift, I gave him his meds and then started feeding him. I don't bother with gloves for feeding since ordinarily, it's not a hazardous activity and gloves aren't dictated for feeding in patients without contagious diseases.
